How we collect, use and protect personal information on nationalmammographyday.org, in plain language.
1. Who we are
This website, nationalmammographyday.org (the “Site”), is operated by Alliance for Health Implementation Science, Inc. (“AHIS,” “we,” “us”), a Delaware nonprofit corporation based in Thousand Oaks, California. AHIS is responsible for the personal information collected through the Site. Questions: info@ahis.org.
2. What we collect
Information you give us. Most of the Site can be used without giving us any personal information. We only collect what you choose to submit:
- Word cloud: your first name, city and state, and how you want to be shown (first name and city, first name and state, or without your name). If you choose first name and state, we don’t collect your city. If you choose not to show your name, we collect only your state. We also record the date and time you joined.
- Facility search: the ZIP code you enter or, only if you tap “Use my location” and allow it in your browser, your approximate location, rounded to about 1 kilometer (0.6 mile). We use it to find nearby facilities and do not store it.
- Email: anything you send to info@ahis.org.
We don’t ask for your email address, phone number, last name or anything about your health beyond what joining the word cloud says (see section 6).
Information collected automatically. We count visits and actions on the Site, such as page views, posts saved and searches, as anonymous daily totals, together with the kind of site that sent the visitor (for example, Facebook, Instagram, a QR code or a search engine). These counts are not linked to you. To prevent spam, when you join the word cloud we create a one-way code from your IP address and the current hour and use it only to limit repeated sign-ups. We never store your IP address itself. Like most websites, our hosting provider keeps standard server logs, such as IP address, browser type, pages requested and the date and time of the request, to operate and secure the Site.
3. What we don’t do
- We don’t use cookies, advertising or analytics trackers, tracking pixels or retargeting.
- We don’t show ads.
- We don’t sell or rent your personal information, or share it for advertising.
- Fonts and images are served from this Site, not from third-party services.
If this changes, we will update this notice first.
4. How we use your information
- To show your entry in the word cloud the way you chose, and to count it in the totals.
- To find certified mammography facilities near the ZIP code or location you give us.
- To reply to your messages.
- To report campaign results in aggregate, for example the number of people who joined and the number of cities and states. These reports do not identify individuals.
- To operate, protect and improve the Site, including preventing spam and removing entries that break our Terms of Use, and to meet legal obligations.
5. Information we publish
The word cloud is public. When you join, your entry appears on the Site right away, shown the way you chose: first name and city, first name and state, or counted without your name. Anyone who visits the Site can see it. We never publish anything else you give us.
Names and places go through an automatic word filter and a check that the city exists in the state you chose. We may hide entries that look false, offensive or like someone else’s information. You can ask us to remove your entry at any time by writing to info@ahis.org with the name and place shown, and we will remove it promptly.
6. Health information
The Site offers general health information and does not ask for your medical history, results or anything about your health. Joining the word cloud says that you had a mammogram this year. If you don’t want that connected to your name, choose “Don’t show my name”; you will be counted in the total without your name. Please don’t include personal medical details in emails to us. We do not use word cloud entries for advertising, for sale, or to train artificial-intelligence models.
7. Service providers
AHIS staff and AHIS contractors support campaign operations. They may use your information only for AHIS’s work on the Site, and they are bound by confidentiality.
We also use a small number of service providers to run the Site. They process information only on our behalf and according to our instructions:
- Netlify, Inc. (United States): website hosting, server logs, and storage for word cloud entries and anonymous counts.
- Microsoft Corporation (Microsoft 365): email used to receive and answer messages.
The facility list comes from the U.S. Food and Drug Administration’s public database of certified mammography facilities. We download it to the Site; your searches are not sent to the FDA.
We may also disclose information if required by law, or to protect the rights, safety or security of AHIS, the Site or others.
8. How long we keep it
- Word cloud entries: for as long as the word cloud is published, plus a reasonable period for our records, or until you ask us to remove them.
- Facility searches: not stored.
- Anonymous counts: kept as daily totals for campaign reporting.
- Spam-prevention codes: used only to count sign-ups within the hour they are created. They can’t be turned back into an IP address and are not linked to your entry.
- Messages: for as long as needed to respond and keep reasonable records.
- Server logs: according to our hosting provider’s standard retention.
9. Your choices and rights
You choose what to show in the word cloud, and you can join without showing your name. You can ask us to access, correct or delete the personal information we hold about you, including removing your word cloud entry, by writing to info@ahis.org. We will respond within a reasonable time and in line with applicable law.
10. Do Not Track and California privacy
Do Not Track. We do not track visitors across websites or over time, and we do not allow third parties to do so on this Site. Because we don’t use tracking technologies, the Site works the same way whether or not your browser sends a “Do Not Track” or Global Privacy Control signal.
California residents. This notice is intended to meet the California Online Privacy Protection Act. We do not sell or share personal information, and we do not disclose it to third parties for their own marketing purposes.
11. Children
The Site is intended for adults. It is not directed to children under 13, and we do not knowingly collect personal information from them. If you believe a child has submitted information to us, contact us and we will delete it.
12. Security
We take reasonable measures to protect personal information, including encrypted (HTTPS) connections and limited access to word cloud records. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.
13. Links to other websites
The Site links to other websites, such as the CDC, state screening programs, the FDA, Google Maps for directions, our supporters and social media. Their privacy practices are their own, and we encourage you to read their policies. When you call or get directions to a facility, you deal with that facility or service directly; AHIS does not receive that information.
14. Changes to this notice
We may update this notice from time to time. The date at the top shows when it last changed. Material changes will be highlighted on the Site.
